They’re exhausted, just like you! It is also the perfect time for you to pass out as well! (2) They have very minimal diapers- after 24hrs, diaper output is a big indicator of how well they’re feeding, since with BF you can’t tell! But on their first full day of life, the day after they’re born, only having one pee and 1 poop is a-okay. The next day, 2 diapers at minimum is great too (3) They may only want mama- you made and literally grew them, once they’re born they are striaght up homesick. All the skin to skin and snuggles is exactly what they want and need.</image:title>

Hand expression in pregnancy (aka antenatal hand expression) is optional and a bonus but not crucial to your success. Hand expression in pregnancy should not be done without a green light from your OB/midwife. Okay now we’re clear that we’re talking how to hand express for use after baby is born. Best case scenario you don’t need to hand express and right after birth, baby latches on, and keeps doing that with good milk transfer at least 8 times a day for 6 months. Prepping freezer meals can be a fun nesting activity if you enjoy cooking, but don’t think this falls on you alone. You know that really nosy great aunt that texts you every other day wondering if the baby is here yet? Her texts may not be helpful, but if she makes a mean lasagna, help yourself and make her feel useful by asking her to make you a tray or two. If family isn’t close by, sign up for services like Lasagna Love to close the gap. Another great idea (because you can only eat lasagna so many times), is to prep crock pot meals in large freezer bags. Add all the ingredients in advanced and write instructions on the bag so when it’s time to cook, you can dump it, set it, and forget it.</image:caption>

Learn about labor and delivery, absolutely, but don’t forget newborn basics, feeding your baby, and postpartum preparation as well. So, where do you start? You might find that your chosen place of birth (i.e. hospital or birth center) offers childbirth education at an affordable cost. It may not be the most unbiased, but it is an option to consider depending on your birth choices. Many doulas, nurses (like us!), lactation consultants, and the like also offer their own private courses that you may find more detailed or tailored to your preferences. Ask mom friends, the professionals you’ve hired, your provider, the local mom Facebook group, or shoot us an email if you’re having trouble finding one.</image:caption>

Utilize the nursery for baby if needed We do not want you falling asleep with your baby in your arms in a hospital bed, chair or anywhere else. The safe sleep seven is very difficult to practice in a hospital safely. You are very exhausted, your support person is exhausted, yet baby doesn't want to be put down. If your hospital has the nursery as an option, take advantage of it if you're comfortable. Rooming in is of course ideal for bonding, breastfeeding and quality sleep just to name a few, BUT we would so much rather you let us know if you can't safely care for baby during your first couple of days postpartum than get into an unsafe situation.</image:title>

The Academy of Breastfeeding Medicine recently released a clinical protocol for mastitis management, and you’ll be surprised with what the latest research has shown. So many techniques that we thought until recently were helping relieve clogs and mastitis, might have just been making it worse. In a nutshell: let’s simplify the management. This means going back to the basics of ice, NSAIDs and breastfeeding on demand/pumping to match feeds, while reducing aggressive management.  Increasing pumps, feeds, using lots of gadgets and interventions can create more inflammation and swelling. It’s all about going back to the core of supply and demand. You demand more, your body makes more.     We have highlighted the latest clogged ducts and mastitis protocols here for you. Some are preventative, others are for treatment purposes. These recommendations are for a healthy mom and baby, where baby is gaining weight and having adequate wet diapers. If you are experiencing frequent mastitis, clogged ducts, please work with a lactation consultant (find one here!) and your OB/midwife at minimum. This is a clue that the feeding/transfer/pumping/breast anatomy may need to be looked into further. If you have any history of breast surgery or breast implants, please also be in close contact with a lactation consultant since mastitis and clogged ducts can look very different for you than others.       <image:caption>Avoid saline soaks or anything topical- yes that haaka in epsom salt can irritate you skin more than it would help with the clog.   Don’t go crazy sterilizing- mastitis is not contagious and not from unhygenic cleaning practices. Clean your pumping parts as you have been (and sterilizing if your pediatrician specifically told you to). No need to clean the nipple before or after feeds. Using nipple cream does not cause a clog/mastitis.  Reduce pain and swelling- manage the mastitis symptoms  - Motrin 800mg every 8 hours  - Tylenol 1,000mg every 8 hours  - Ice- every hour or more frequently  - Heat- opens the milk ducts. It can worsen symptoms or provide relief, depends on the mother  - Sunflower lechtin has been known to reduce inflammation and emulsify milk. Manage oversupply- having an oversupply predisposes one to breast inflammation and milk duct narrowing. Aim for the basics of supply and demand (EPers- if your goal is to EP for the shortest amount of time possible and want an oversupply, keep it going if you aren’t  getting frequent clogs/mastitis!)</image:caption>

We did well with latching, but my “barely there” supply for my toddler was still a very overwhelming supply to a brand-new baby. We dealt with a tremendous amount of spit up in the first couple of days as a result. I found that tandem nursing was my saving grace for this situation because the toddler could take away some of that overwhelming volume from the baby. I found it easiest to latch the toddler on one side and the newborn on the other, or sometimes to even let the toddler go first until the letdown subsided, then replace her with the newborn on that side. Tandem feeding was also my secret weapon for preventing a lot of new sibling jealousy in that adjustment period.</image:caption>

Because I gave birth in the hospital I worked at, I knew that one of the lactation consultants I worked with had a weekly breastfeeding support group every Thursday evening. The week my daughter was born, Thursday ended up being December 26th. I decided to pack my baby into the car and let my mom drive us up to the hospital the day after Christmas because I was beginning to feel my milk come in and panicking about engorgement. When I got there, I was the only person who came for support group, so I basically got 90 minutes alone with an IBCLC (and dear friend) who doted on us, encouraged us endlessly, rubbed my shoulders, and taught me a million different tricks and positions for latching her.</image:caption>

      <image:caption>When the door to my room opened next, JoAnn AND Lisa walked in the room. Again, I was shocked and excited to see them both! They were in the middle of switching off when my nurse called them, so they decided to come together. JoAnn made a remark that they didn’t get to attend births together often anymore and it was a special treat to do so. My nurse then finally emptied my bladder, my water immediately broke, and it was go time. With my nurse on one side, Lisa on the other, JoAnn at my feet, and my husband behind my head for moral support, we began pushing. Yes, I pushed the entire time in my mask and again, I didn’t even think to do otherwise. I hardly noticed it and it truly wasn’t as terrible as you might expect. I have a habit of always putting a few drops of essential oil in my masks, which may have helped my comfort level. On this day, it was a eucalyptus blend to help me breathe. The midwives offered to let my husband catch her, but he declined (he regrets that now). About 20 minutes later my daughter was crowning, direct OP, and thrashing her head against my pubic bone. There isn’t an epidural in the world that could’ve saved me from that horrific feeling and in one more frantic push to make it stop, Madelyn was born at 8:34am.</image:caption>

      <image:caption>The story of my second daughter really starts way back at my six-week postpartum midwife appointment after my first pregnancy. My husband and I knew we wanted to have our children relatively close together, so I made sure to ask how long we had to wait in order to safely conceive again. While 18 months would’ve been ideal, she let me know that 12 months would still be ok if that’s what I wanted. Fast forward 6 months to our daughter sleeping well at night, moving out of our room, and my baby fever returning. We cast the caution to the wind thinking it would take us many months to conceive just like the last time. Spoiler alert: it didn’t take many months. I was 7 months postpartum and about to take on another pregnancy followed by raising two children under two years old. We were thrilled, but also apprehensive. Turns out my body just really doesn’t like being pregnant and the first miserable experience wasn’t a fluke. Despite extensive chiropractic work, I was exhausted, sick, in pain, and trudging through life with a rambunctious one year old for my entire pregnancy. Here’s a photo of me about 8 months pregnant – a photo I only asked for because I realized I didn’t have any photos of my belly this time. The proof of mom photos were also few and far between because while she was at the park with dad, I was laid up on the couch most days. In this instance, thank goodness for pandemics and shutdowns that allowed my husband to work from home. I wouldn’t have made it through otherwise.</image:caption>

      <image:caption>She let out a quick little shriek, then promptly fell back to sleep on my chest. JoAnn tried to convince me that she had given me “all the hell she was going to” now that she had finished out the world’s most miserable pregnancy with thrashing her way through my pelvis and settling peacefully on my chest. I can tell you over a year later her favorite way to wake me up in the morning is still with a swift little headbutt. I got the perfect “Golden Hour” this time around and for two hours, she never left my chest – not even for a diaper, which I suppose makes me brave. Eventually she was taken to the warmer next to me for a weight and a small assessment. At 8lbs 4oz, she was my peanut. She had some significant bruising on her forehead from the sunny side up position she arrived in. You can tell that she had her brow furrowed when she was head banging her way out by the stripe pattern of the bruises. We started breastfeeding right away and were quite successful. Stay tuned for my postpartum story that includes how I navigated feeding a newborn a full milk supply (I was still nursing my toddler). There’s a lot of spit up in that story.</image:caption>

There’s family to see, festivities to celebrate, and shopping to do for most of the month. I wondered if she would arrive on her own special day or overlapping a memorable event. My due date was Christmas Day. Pregnancy was physically hard on me and I was excited for the part I was accustomed to as a postpartum nurse. I knew I’d thrive better there. I don’t have many photos of myself while pregnant, but here is the last one I have with my nephew, about two weeks before he got to meet his new cousin and best friend.</image:caption>

      <image:caption>While they were gone, I enjoyed an ice pop and Lisa stitched up my second-degree tear. If you had asked me in the moment if I was disappointed that I lost my golden hour, I’d tell you yes, of course. But with a little bit of hindsight perspective, I can tell you that it happened the way it was meant to and I don’t regret it. I do sometimes wish I had spoken up more in the moment. Maybe she wouldn’t have left me. I don’t really remember her breathing or color being is poor as they said, and when I look back at photos, she looks pink and comfortable to me. But I also recognize that I wasn’t the nurse in the room and my judgement might not have been the best that day. Mostly I just remember that my husband returned with my daughter absolutely beaming. He got a special few moments alone with her and proudly got to present her back to me a little while later. I would never steal that moment from him even if I could go back and redo things. In the end, I had uninterrupted skin to skin with her for the next several hours, initiated breastfeeding, and we made up for it all and then some.</image:caption>

It’s ok if you feel a new incredible level of love and affection for your partner. It’s ok if you want to scream every time they sneeze loudly. It’s ok if you feel all of these things in the span of five minutes. The fourth trimester is easily the biggest rollercoaster of physical and emotional growth we will ever experience as women. Let’s decide to do it hand in hand. No matter how we feel, no matter what our choices are, no matter what our journey looks like, we’ll make it to the other side. Together.</image:caption>
